- Participation Requirements: 75% Participation
- Ineligible Industries:
- PEOs
- Casinos
- Nursing Homes
- Long Haul Trucking
- Employee Leasing Firms
- Hotels
- Indian Tribes
- Bars
- No outside HMO involvement
- No Indemnity plans
- Groups with over 20% over age 60 will be declined
- No Manual quotes on groups over 150 lives
- Current rates & renewal rates if available are required along with the current Schedule of Benefits
- Claim experience is required on groups currently self-funded or if experience is made available to employer
