COBRA is a federally mandated continuation of coverage that applies to most Employees and dependents covered by most fully insured and self-insured medical plans. This law can allow an individual to continue their medical coverage when they no longer meet the eligibility requirements of a Plan. - What documentation is required for individuals who are on a COBRA continuation?
- A copy of the signed and dated COBRA continuation election form completed by the individual involved
- An explaination of the circumstances surrounding the indiviudal's COBRA election. This may be addressed in the COBRA continuation election form
- Documentation that the individual made timely COBRA continuation payments.
- Other medical coverage documentation from the individual involved confirming whether or not they have or had other coverage during the COBRA continuation time period.
